What information is needed for a child's Change of Name deed?

A number of things starting with the agreement of those with Parental Responsibility for the child - usually this is the child’s mother. A named father on the birth certificate also needs to give their agreement too. Rights can be gained or lost – for example by adoption. Whatever the situation, if more than one person has Parental Responsibility, you will need to provide a signed letter of agreement from all concerned.

You will personally need to provide proof that you have Parental Responsibility, for example the child's full birth certificate which shows you named as a parent - or adoption papers or a Court Order giving you Parental Responsibility.

  • Proof of your identity
  • Proof of your address
  • Details of the planned change or changes
  • And finally we need the reason for the change
